Grant Guidelines - This format is to be used for the July 1, 2008 summer grant cycle.
This document identifies the
general guidelines applicable to the Foundation’s grant making process. The Foundation may change these guidelines,
or make grants that do not fall within the range of these guidelines at its
discretion. Grants are made once a year to eligible organizations for projects that support the Foundation’s grant criteria.
The Foundation is interested
in raising public awareness of oral health issues and encourages applicants to
make this a part of any funded project through media communications and
community participation.
Eligibility
To be eligible for funding
consideration, an organization must be a:

Public
charity with tax-exempt status under section 501 (c)(3) of the Internal Revenue
Code, or
·
Public
or governmental entity such as a school or health district, or
·
Tribal
organization with 7871 tax-exempt status
Organizations applying for a
grant must have a physical presence in the state of
Examples of projects that
are not eligible for grants are:
·
Administrative costs including any and all salaries
·
Fundraising events
·
Individual treatment costs
·
Lobbying
·
Ongoing
programs, ongoing general operating expenses or existing deficits
·
Political organizations or political campaigns
·
Projects not related to oral health
·
Projects outside the state of Kansas
Application Format
Applicants should include the following 11 items. Please limit your
application to five pages plus the required attachments.

Application Process
Grant applications are due on or before July 1, 2008 by 12:00 Noon in the Overland Park office. Funding decisions are scheduled to be
announced during August, 2008. To be considered for funding, applicants must complete and submit a proposal with the requested attachments. INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.
Type of Funding Provided
Funding is
provided for new initiatives, new components of existing programs,
start-up funding or one-time events that include one or more of the following Foundation grant criteria: prevention, significant/large impact, or sustainable solutions. Grants will be limited
to projects related to the advancement of oral health.

Major dental equipment
·
Dental supplies to safety net clinics
·
Toothbrush kits
·
Dental educational materials
·
Orthodontic care
·
Xylitol wafers
·
Fluoride varnish
Copies Required and Attachments
Send one original and ten copies of your application, each secured with a binder clip, along with a cover letter signed by the highest-ranking official in your organization and all the required information below.
·
Grant Application.
·
List of board members and their affiliations.
·
Letters of support from your partners.
·
Any other information you consider pertinent to your application.
Also include one (1) copy of each of the following:
·
Current detailed operating budget for the organization and most recent Form 990 (with
attachments).
·
Most current audited financial statement. If
not available, a recent balance sheet (assets and liabilities) and a statement of income and expenses.
·
Current copy of the applicant's U.S.
Treasury (IRS) tax determination letter.
Submitting the Application
Grant application and copies should arrive at our Overland Park office no later than 12:00 Noon on
July 1, 2008. The Foundation will have only one grant cycle in 2008.

If you have questions concerning your organization’s eligibility for funding consideration, funding criteria or review process, OR, if your request is over $100,000 please contact Executive Director, Karen Finstad at 913-327-3727, to discuss details before you begin the application process.
Application Review
Applications will be
reviewed to establish the significance of the applicant’s
proposed activities and to make recommendations to the Foundation Board
regarding the consideration of the application and general terms and conditions
of said grant. The Foundation may also
wish to conduct screenings and/or interviews and may ask the applicant to
provide additional information including, but not limited to, letters of
recommendation, references, and the like.
Upon approval of the grant
application final terms of the grant will be negotiated
and a Grant Agreement will be prepared which outlines the terms of the grant.
Grant Monitoring
The Foundation will monitor
the grantee’s activities during the duration of the grant, and will require
periodic written progress reports to ensure that the grantee is
in compliance with the terms of the grant.
