Grant Guidelines - This format is to be used for the July 1, 2008 summer grant cycle.  

 

This document identifies the general guidelines applicable to the Foundation’s grant making process.  The Foundation may change these guidelines, or make grants that do not fall within the range of these guidelines at its discretion.  Grants are made once a year to eligible organizations for projects that support the Foundation’s grant criteria.

 

The Foundation is interested in raising public awareness of oral health issues and encourages applicants to make this a part of any funded project through media communications and community participation.

 

 

Eligibility

 

To be eligible for funding consideration, an organization must be a:

 

·      Public charity with tax-exempt status under section 501 (c)(3) of the Internal Revenue Code, or

·      Public or governmental entity such as a school or health district, or

·      Tribal organization with 7871 tax-exempt status

 

Organizations applying for a grant must have a physical presence in the state of Kansas .  The project for which funds are requested must benefit people living in Kansas .  Priority will be given to proposals that:  emphasize prevention; improve access to dental services; have measurable improvement goals; show strong community support for the project.  Grants will be limited to projects related to the advancement of oral health.

FOUNDATION GRANT CRITERIA


The focus of the Delta Dental of Kansas Foundation is on grants and programs which emphasize prevention, have significant and/or large impact, and are sustainable solutions.

Prevention:
  • Intervention with early care
  • Partnering with existing networks
  • Educational information/support and/or providing tools for positive behavior (i.e. toothbrush kits)

Significant and/or Large Impact:
  • Expanding services and collaborations that will affect large numbers of people preventing the need for extensive dental care
  • Intervene in dramatic oral health care needs for underserved individuals

Sustainable Solutions:
  • Building a trained workforce
  • Providing a steady reliable source of support (i.e. dental safety net clinics)
  • Infusing new providers in existing networks

 

 

Examples of projects that are not eligible for grants are:

 

·      Administrative costs including any and all salaries

·      Fundraising events

·      Individual treatment costs

·      Lobbying

·      Ongoing programs, ongoing general operating expenses or existing deficits

·      Political organizations or political campaigns

·      Projects not related to oral health

·      Projects outside the state of Kansas

 

 

Application Format

 

Applicants should include the following 11 items.  Please limit your application to five pages plus the required attachments.

 

  1. Application cover page must be completely filled out with contact information for organization, executive director, project manager and your organizations' website. (Fill out application cover page).

  2. Funding request (total funding required for the project and the specific amount requested from Delta Dental of Kansas Foundation).

  3. Project summary (100 words or fewer).

  4. Organization description (history, mission, services provided).

  5. Project description (include objectives, activities and a timeline which includes the starting and completion dates).

  6. Description of need and how the project addresses that need (document the need for the project and describe the anticipated impact on systems of care, Fill out demographic page; who and how many people will benefit; and the geographic scope of the area to be served).

  7. Budget (detailed expense budget for the project with the proposed use of Delta Dental of Kansas Foundation funds highlighted; description of committed and potential revenue sources and amounts) (Fill out attached budget & revenue page).

  8. Leadership and partners (names and qualifications of staff responsible for project implementation).   If a collaborative effort, identify project partners, the role played and contribution made by each.

  9. Sustainability (describe how the project will be sustained after Foundation funds have been expended).

  10. Evaluation plan (define project success and describe how effectiveness and outcomes will be measured).

  11. Acknowledgment plan (describe how the Delta Dental of Kansas Foundation’s grant will be acknowledged).

 


INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED

 

Application Process

 

Grant applications are due on or before July 1, 2008 by 12:00 Noon in the Overland Park office. Funding decisions are scheduled to be announced during August, 2008. To be considered for funding, applicants must complete and submit a proposal with the requested attachments. INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.

 


Type of Funding Provided

 

Funding is provided for new initiatives, new components of existing programs, start-up funding or one-time events that include one or more of the following Foundation grant criteria: prevention, significant/large impact, or sustainable solutions. Grants will be limited to projects related to the advancement of oral health.

 

 

Types of Grants Funded

 

·      Major dental equipment

·      Dental supplies to safety net clinics

·      Toothbrush kits

·      Dental educational materials

·      Orthodontic care

·      Xylitol wafers

·      Fluoride varnish

 

 

Copies Required and Attachments

 

Send one original and ten copies of your application, each secured with a binder clip, along with a cover letter signed by the highest-ranking official in your organization and all the required information below.

 

·      Grant Application.

·     List of board members and their affiliations.

·     Letters of support from your partners.

·     Any other information you consider pertinent to your application.

 

 

Also include one (1) copy of each of the following:

 

·      Current detailed operating budget for the organization and most recent Form 990 (with attachments).

·      Most current audited financial statement.  If not available, a recent balance sheet (assets and liabilities) and a statement of income and expenses.

·      Current copy of the applicant's U.S. Treasury (IRS) tax determination letter.

 

 

Submitting the Application

 

Grant application and copies should arrive at our Overland Park office no later than 12:00 Noon on July 1, 2008. The Foundation will have only one grant cycle in 2008.

Application Review

 

Applications will be reviewed to establish the significance of the applicant’s proposed activities and to make recommendations to the Foundation Board regarding the consideration of the application and general terms and conditions of said grant.  The Foundation may also wish to conduct screenings and/or interviews and may ask the applicant to provide additional information including, but not limited to, letters of recommendation, references, and the like.

 

Upon approval of the grant application final terms of the grant will be negotiated and a Grant Agreement will be prepared which outlines the terms of the grant.

 

 

Grant Monitoring

 

The Foundation will monitor the grantee’s activities during the duration of the grant, and will require periodic written progress reports to ensure that the grantee is in compliance with the terms of the grant.
